A valid credit card number is required for booking an appointment. You can choose to use another method of payment; cash, check, Paypal and Zelle® are welcome. After our appointment you will be provided with an electronic copy of your superbill to submit to your insurance for reimbursement.
A valid credit card number is required when booking an appointment. Your card will not be charged unless you cancel your appointment without a minimum 24 hours notice, at which point a $50.00 cancellation fee will be automatically applied.
Protecting your family is important to me. I take special care to disinfect all equipment before and after your visit. I wear a surgical mask during the duration of the visit, with the exception of momentary removal if needed to drink water. For everyone’s safety, you are required to wear a mask as well. If I, or anyone in my family, has any symptoms of illness, I will be in touch with you to either cancel or reschedule our appointment. I ask that families contact me prior to their appointment if anyone in the family shows any signs of illness.
Texting and email communication as per Hipaa regulations is done thru a secure, private platform. Once you book your appointment all communication will take place through secured communication. Information will not be shared with third parties without your prior authorization.
